STEPS & CHECKLIST

Note: Application for Type ‘A’, ‘B’, ‘C’ i.e. paid service are to be submitted at BLS. Application for Type ‘D’ i.e. gratis service can be applied and obtained online without submitting at any office.

Type A (Paid)

1. Once a new passport is issued after completing 20 years of age

1. Printout of duly filled online Application Form (All documents are mandatorily required to be uploaded.)
2. Valid Foreign Passport & its copy.
3. Current OCI Card in original and copy
4. Registered Marriage Certificate (if married)
5. Copy of spouse’s current passport (if married)
6. Professional details of the applicant
7. Mandatory Declaration: CLICK HERE
8. Proof of Address – (any of the following: (a) Driving license,(b) Photo ID issued by the Provincial Government,(c)Utility bills (Gas, Electricity, Water or Landline Telephone Bill displaying applicant’s address/House Lease Agreement).(*Phone bills, Bank statements are not accepted as valid address proofs.)
9. Photograph (as per specification)

2. In case of change of personal particulars viz. name, parent's name, nationality, gender etc.

In addition to documents mentioned in Type A1 (1) to (9)

1. New passport reflecting the changed gender.
2. Old passport.
3. Medical certificate
   1. If issued from India, copy should be attested by Attestation Cell, Ministry of External Affairs, New Delhi.
   2. If issued by any other country which is member of The Convention of 5 October 1961 Abolishing the Requirement of Legalisation for Foreign Public Documents (HCCH 1961 Apostille Convention), copy should be apostilled by competent authority from that country.
   3. If issuing country is non member of Hague Convention, 1961, attested by the competent authority concerned followed by re-attestation by the Indian Embassy there.

3. In case of loss/damage of OCI card.

In addition to documents mentioned in Type A1 (1), (2), (4) to (9)

1. In case of Lost OCI, a police report bearing lost OCI number is required.
2. In case of Damaged OCI, old damaged OCI booklet is required.

4. In case of Foreign Spousal Application (with 5 year validity)

1. Original OCI Card
2. All documents as per foreign spousal category checklist (link for spousal category checklist)
3. Joint Self-Declaration of Subsistence of Marriage

Type B (Paid) Online updation of Passport Details

In case of Updation of passport details after 3 months of issuance

1. Email acknowledgement received from: click here

3. Latest Passport Copy

Type C (Paid) Voluntary Surrender of OCI Card

In case of surrender of OCI Card Please fill online form and submit at BLS Centre

Type D (Gratis) Online updation of Details

Application link

1. Every time new passport is issued (Type A1)
   1. Upload new passport
2. Change of address/occupation/contact details
   1. Upload Proof of Changed address/occupation details
3. Foreign spousal category which has lifetime validity,
   1. Upload new passport
   2. Declaration for subsistence of marriage
   3. Valid Indian passport or OCI card
   4. Valid foreign passport of the Indian-origin spouse.

Photo Specifications

1. One color photo as per below specification is required to be submitted along with the application:
2. Size – 2 inch x 2 inch (51 mm x 51 mm)
3. Background – Background should be plain white colored without borders.
4. Clothes – Dark color
5. Eyes – Open

NOTE:

1. After online submission of application form, no changes can be made manually / hand writing. In case of any error / changes, please fill a new online application form and use that for further submission to BLS.
2. Consulate cannot edit or make any changes to the applicant’s online application form.
3. Consulate reserves the right to call for additional documents, as considered necessary.
